- Please contact the center director in your area to confirm availability.
- Complete all the information requested on page 2-5 of the pre-enrollment form
- Email or fax the completed pdf form to the appropriate center.

Please note that center email addresses are provided on page 6 of the pre-enrollment form. You may print the completed form and mail or bring it into one of the centers, or you may fax your completed forms to the appropriate center using the phone numbers listed on page 1.
